Some adjustments may be small while others may signify a complete transformation. A limited number of items are listed in this section to assist you in accessing benefits, which may be available to you.

- Should you decide to apply for the disability extension of coverage, please complete the Disability Application Form and have the Attending Physicians Statement completed by your doctor and return them to the Administrative Office along with any supporting documentation. You will be notified of the decision in writing.
- If your disability is a Terminal Illness, review the Terminal Illness section to review the benefits available.
- You should check your vesting status under the Pension Plan to determine whether you are vested for a pension benefit. If you are not vested with the Pension Plan, your disability may affect your Qualified YearsA Plan Year in which the Participant earned at least eight Credited Weeks towards vesting, depending on the duration of your disability.
