See your primary care doctor as often as you like at no cost, skip the deductible entirely, and know what every visit costs before you walk in.

Three steps, then you’re set

Getting care under the Centivo Plan comes down to one habit: pick a primary care doctor, and start there.

    Pick your doctor

    Everyone you cover chooses one from the Centivo Network – a pediatrician for kids, or free virtual primary care through Centivo Care for anyone 18 and up. Change whenever you want.

  2. 2

    Start there for everything

    Checkups, new symptoms, ongoing conditions. Those visits cost nothing, so there’s no reason to put one off, which is how the plan keeps the rest of your costs down.

    Referrals happen for you

    Need a specialist? Your doctor submits the referral, it’s approved automatically, and it’s good for a full year. It shows up in the Centivo app and you book from there.

You’ll recognize the names

Centivo contracts directly with local health systems rather than renting a national network, which is where the lower rates come from. Many providers carry over from The Industry Health Network you use today.

Centivo isn’t available everywhere

The plan is offered only in certain service areas, so your home ZIP code has to fall inside one to enroll. Check yours before you decide, and while you’re there, search the network for the doctors you already see. Staying in network is where the savings live.

Three things you don’t have to sign up for separately, add on, or pay extra to use.

  • Centivo Care

    Virtual primary care with same- or next-day appointments, free. A dedicated team handles screenings, refills and new concerns, and can be your designated doctor if you’d rather not have an in-person one.

  • MDLIVE

    Virtual urgent care and behavioral health, around the clock, also free. For the sore throats and sinus infections that never happen during office hours.

  • The Centivo app

    Pick or change your doctor, find in-network providers, track referrals, pull up your ID card, and message Member Care without sitting on hold.
